Colorectal Screening

• Home-based FOBT in the Past Year, No Colonoscopy in Past 10 Years, Ages 50-75
• Had Colonoscopy in Past 10 Years, Ages 50-75
• Guideline Sufficient CRC, Ages 50-75

Women's Health

• Mammogram in Past 2 years, Ages 50-74
• Mammogram in Past 2 years, Ages 40+
• Pap Smear in Past 3 Years, Ages 21-65

Smoking

• Current Smoking, Ages 18+
• Former Smoking, Ages 18+
• Former Smoking, Quit 1 year +, Ages 18+
